Effective Date of Current Policy: December 06, 2005
Future of Music Coalition ("FMC") is committed to protecting
the personal information you submit to us online. This Privacy
Policy describes our practices regarding the collection and use of information
submitted through our U.S. website, http://www.futureofmusic.org (the "Site"). By
using our Site, you agree to the collection and use of information as
set forth in this Policy. If you do not agree to this Policy, please
do not use the Site. This Policy is the sole authorized statement
of our practices concerning the collection, use and protection of personal
information through this Site.
We may update this Policy from time to time. We will notify you
of material changes to this Policy by posting the updated Policy on this
page. We ask that you bookmark and periodically review this page
to ensure your familiarity with the current version of our Policy. You
can determine when this Policy was last reviewed by checking the "Effective
Date of Current Policy" line at the top of this Policy.
The Types of Information We Collect Online
Our Site collects personal information such as names, postal addresses,
email addresses, telephone numbers, and other information, such as survey
responses. If you make a donation or purchase on our Site, we may
collect your credit card information as well as other information related
to your donation or purchase. Like many websites, our Site logs
tracking data, which may include information such as the IP address of
the visitor’s computer, its browser type and operating system,
the referring site, which pages of the Site were visited, the order in
which they were visited, and which links were clicked.
FMC periodically conducts online surveys on the Site, which help us
to assess the interests and objectives of the musicians and citizens
who use our Site. While our surveys may request demographic data
such as age, gender, and household income to help us better evaluate
the results, we do not correlate this demographic information with the
identity of the individual who provided it. In some cases, we may
give survey participants the option of requesting an emailed copy of
our final survey results. The email address submitted by the survey
participant for this purpose is not correlated with his or her survey
responses. Survey participants are not required to provide an email
address in order to participate in our surveys.
Our Site facilitates individual participation in policy-based actions,
such as the submission of public comments to the FCC or other government
entities. At times, the Site may provide a mechanism allowing individuals
to submit public comments to the FCC by using a comment form provided
on the Site. The form identifies the topics on which public comments
are requested and is intended to help participants craft their responses. Submitting
public comments to the FCC requires the participant to provide his or
her name, address, and email address. Public comments submitted
through our Site are automatically transferred to the FCC. FMC
does not maintain copies of the comments submitted to the FCC through
our Site, and we do not use contact information provided in public comments
submitted through our Site to communicate with those individuals.
FMC sponsors the Health Information Navigation Tool ("HINT"),
which helps to provide musicians with basic information about obtaining
health insurance coverage. Our Site allows you to schedule a telephone
appointment with a HINT representative. In scheduling your telephone
appointment through the Site, you will be asked to provide your first
name; your city, state, and zip code of residence; the telephone number
to be used to reach you for this appointment, and your email address. You
may also elect to provide other types of personal information through
the Site, such as your age, gender, marital status, number of children,
and information about your income and current insurance coverage. This
information helps the HINT representative begin to identify types of
coverage that may be available to you, and helps us to understand the
needs of HINT users. Please refer to our Terms of Service for
other important information about HINT.
Our Site is not directed to children and we do not knowingly collect
personal information from individuals under eighteen years of age.
How We Use and Disclose Information Collected Online
We generally use the information we collect online for the purposes
for which it is provided and to enhance the products and services we
offer to you. For example, we use your personal information to
complete transactions that you enter into using our Site, such as requests
to make a donation or purchase, subscribe to our e-newsletters, or register
for our events. Our e-newsletters are sent only to individuals
who have subscribed for them or otherwise requested to receive them. If
you would like to receive our e-newsletters, please
sign up at this page.
We also use your personal information to communicate with you about
specific transactions, to make sure that you are satisfied with our products
and services, or to find out how we can serve you better. You may,
from time to time, receive information from us regarding events, policy-based
actions, new products, services, or features we think you will find valuable.
HINT is operated with the assistance of third-party insurance industry
professionals ("HINT Representatives"). Therefore,
any personal information that is submitted for purposes of scheduling
a HINT telephone appointment may be shared with HINT Representatives.
We work with third parties who provide services that may include, but
are not necessarily limited to, website hosting, data processing and
analysis, newsletter transmission and distribution, back-up and security
services, and processing of credit card transactions. We may share
personal information with these service providers for the purpose of
providing these services. Although we seek to require these service
providers to protect such personal information, and to use it only for
the purpose for which it was provided, we cannot be responsible for third
parties’ violations of this Policy or of any other rights of visitors
or consumers that use our Site.
We will provide personal information to law enforcement and other entities
when we determine that such disclosure is required by subpoena, warrant,
court order, or to protect the interests of FMC or the individuals who
use our Site or services. We also may provide personal information
and other information collected online to a successor entity in the event
of a sale of our business or assets.
Security
We take measures designed to prevent unauthorized intrusion to the Site
and the alteration, acquisition or misuse of personal information. Unfortunately,
however, no security system or system of transmitting data over the Internet
can be guaranteed to be entirely secure. Thus, while we seek to
protect your personal information, we cannot be responsible for loss,
corruption, or unauthorized acquisition of personal information furnished
to this Site or for any resulting damages.
Third Party Websites
Our Site may include links to other Internet websites, including sites
that we do not own or operate. Unless otherwise expressly stated,
we are not responsible for the privacy practices or the content of such
websites, including such sites’ use of any information collected
when Site visitors click through links to those sites. If you visit
such websites, we encourage you to check the privacy practices of the
sites.
Offline v. Online Practices
This policy applies solely to our collection, use and disclosure of
personal information obtained online through our Site, and does not apply
to any of our practices conducted offline.
